What To Know Before You Visit

Your Initial Visit

Personalized, gentle, and comprehensive patient care is our number one priority.  Andrea will schedule your appointment as promptly as possible.  You will be able to complete your necessary forms through this website by clicking here, only requiring signature upon arrival and check-in with Andrea.  If you have pain or an emergency situation, every attempt will be made to see you that day.  We try our best to stay on schedule to minimize your waiting.  Due to the fact that Dr. Sullivan provides many types of endodontic services, various circumstances may lengthen the time allocated for a procedure.  Emergency cases can also arise and cause delays.  We appreciate your understanding and patience. 

Number of Visits

Your initial visit can be scheduled as a consultation with our without treatment or you may be coming in on short notice as an emergency.  Most of our appointments are scheduled as consultation with treatment, requiring only a single visit, but there are certain instances such as sedation, surgical procedures, emergencies, or teeth of a complex nature where consultation requiring a later appointment might be necessary.  Lastly, certain teeth with significant infection may require two visits, an interim medicament put in the tooth with the first visit and a second visit required for removal of the medication and completion of the root canal.  

Items to Bring With You

To expedite your visit, having the following items with you makes your visit go smoothly.

  • A list of medications you are taking

  • Your medical/dental insurance cards and documentation

  • Your referral slip from a general dentist

  • A photo ID

  • X-rays (if your general dentist took these recently). 

  • A parent or guardian (if patient is under 18 yrs.)

 

If you recently had an x-ray, you can ask your dentist to send them to our office.  If you have had a CBCT (3D scan) taken of the tooth recently, you will need to bring the disk provided by your dentist as these files are too big to email.  If you do not have any recent x-rays or we need additional views of the tooth, we can easily take these in our office during your visit as well as a CBCT scan.

Medical Information

Regarding medication, please do not stop taking medication prior to your visit, but continue as recommended by your physician.  We also ask that you eat meals as normal, especially if you are diabetic or taking insulin.  If you have a medical condition that may cause concern during your procedure (MVP, heart condition, high blood pressure, etc.) we ask that you let us know in advance of your visit so we can make necessary medical consultations with your doctor to be better prepared.

 

If your dentist or physician has suggested antibiotic pre-medication for a medical condition, please make sure you are taking the appropriate antibiotic the day and time of your procedure

Financial Policy & Payment Information

As a courtesy to you, we will electronically file your insurance claim and have them reimburse you directly as this will shorten the turnaround time for you.  Please keep us informed of any changes to your insurance plan.  You are responsible for the fees charged by our office, no matter what your insurance coverage may be.  Most insurance companies should respond to the claim within four to six weeks.  For your convenience we accept Visa, MasterCard, American Express and Discover.  We also participate with CareCredit:  Apply online at www.carecredit.com or in our office.  We offer six and twelve months no interest payment plans. 

 

We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.  If you have questions regarding your account, please contact Andrea at 941-840-1906, many times a simple telephone call will clear any misunderstandings.  Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
